Can anyone give me advice on this one. I have a fantastic candidate who is absolutely perfect for a particular position. The client thinks this as well. I've proceeded with references and have given them two excellent references from the candidate's previous employer. My clients wants to do a reference with his current employer and have presented him with a conditional offer.

I know this is standard practice with some companies and have explained that to the candidate. Of course he is beyond nervous and I get the feeling that he is contemplating declining the offer.

Any advice on how to go about making sure both my client and candidate are happy?

Has he accepted the position and signed an offer letter? The offer should be contingent upon successful completion of references and background check. Perhaps, he is still on the fence about your offer and is unsure - has he given notice and identified a start date?
